XB1 Flak Glitch

I've enjoyed many hours of game time, playing through the main campaign (twice), and am currently working my through all of the DLC's. However, I have recently come across an irritating bug concerning the colonial ships' flak system.

During the last few missions of the Ghost Fleet campaign, I would initiate any given ship's flak defenses normally, as I have throughout the game. However, I've suddenly discovered that, once I turn the flak off for any reason (such as firing a salvo of munitions, switching flak to the opposite side of the ship, etc), I no longer have any flak at all when I try to turn it back on. The flak coverage highlight area no longer appears to tell me which side of the ship that the flak field should be in operation, and if I try to initiate the flak anyway, I have no flak sounds or graphics on screen--even though the ship's menu tells me that flak is on. Worse, I am now completely open to any enemy salvos fired at me, and the severe damage that follows. I found that I can even fire my own missiles, torpedoes, etc., through my now non existent flak field, unimpeded. This game breaking issue continued through the final couple of missions of Ghost Fleet, and the problem has continued to persist now that I have started the Armistice campaign. In effect, I now have a one-time use flak defense system for all ships on all of my fleets.

I've tried a cold reboot of the console to clear the system's cache. When that didn't work, I uninstalled the game and all DLC's, rebooted the console and did a complete reinstall, and yet the problem continues.

Is this a known issue? And, more importantly, how do I rectify the issue to be able to finish out the BSG saga.
