This bug has been annoying me ever since, but I've always neglected to report it, it seems:
US Pacific DLC, Pearl Harbor scen:
If there is an unit in the airspace over the "Ford Island" airfield hex, the aux. Warhawk fighter plane will NOT spawn:
(This may be true for other airfields as well, but there're usually no enemy planes)

Same error, different map: Marines DLC, IwoJima.
The reward aux. "Mustang" will not spawn if there's an air unit above the airfield hex. So this ain't no scen bug, but a trigger error, it seems.

This is strange as I have tested that spawning units, in particular planes, can also spawn around the other adjacent six hexes if the original hex is occupied.
Only if all seven hexes are occupied then the spawn will puff to nothingness.
PS: doesn't really matter if allied or enemy planes occupy hexes, as long as there is at least a single hex empty.
Spawns at map edges can surely be evilishly exploited by occupying much better than in the middle of a map.

Yeah, I have the same impression (usually), but at least in those two instances, the spawning error is reproducible.
Maybe it's the trigger configuration. Like: spawn & event-message = bug? I don't know, thus posting it here.

No idea, but I can't get this reproduced.
I ran the scenario from the editor.
It's my modified one, but I'm pretty sure I have absolutely not touched these triggers.
I set all AITasks to Idle and placed a jap recon plane above each of these three airfields for a test.
These allied planes seem to spawn fine next to the airfields.
I don't think I really need to place many more aircraft around the airfields for another test.
Try again like I tested it. Shouldn't take much time.

I'm afraid this issue is becoming an X-file.
I've removed all land units below just to be sure.
While the plane can spawn anywhere on a free hex on my regular test map, it can't anymore on Pearl and tiny-map if the hex of the spawn point and the upper-right one is occupied. Any other hexes can be free but are ignored.
On my tiny map, land-unit spawns could still evade to other added free land-hexes until all land-hexes were occupied. Then the spawn failed, so no spawn on water hexes in naval transports.
At least this can be kept in mind now that these two hexes should be free for spawns to trigger.
