What is the rule regarding heavy cavalry fleeing from LH charging them in the flank when the are not in combat?

In my experience, always the exact opposite of what I want to happen. XD

It may have flank threatened. If so, they have large probability to evade. Besides, true heavy cavalry never flee( Looking at cataphract).

The rule is that they don't automatically evade (unless there is another unit of enemy non-light troops that could charge their flank/rear this turn). They may still evade if they don't fancy their chances, particularly if they have a good chance to outdistance the light horse.

See patch notes v1.2.5
o Cavalry, camelry and chariots will no longer automatically evade if charged in flank/rear by light horse.
And v1.5.12
o Evady Cavalry/Camelry/Light Chariots will no longer allow themselves to be pinned by a light horse charge if they are also threatened with a possible flank/rear charge by non-light troops this turn. This was an unintended consequence of the previous change to prevent Cavalry/Camelry/Light Chariots from automatically evading from flank/rear charges by Light Horse. Note that it is still possible to pin them with light horse that start the turn in an adjacent square if the heavier charge threat is frontal.

Will that happen if the heavy cav are pursuing another unit? Or only if they are idle? What if the evade paths are obstructed by other units and forest (the only open non-unit spaces are forest)?

Units can't evade while pursuing.
What if the evade paths are obstructed by other units and forest (the only open non-unit spaces are forest)?
They don't take obstructions into account when deciding whether to evade. If they do evade, they will turn, fail to move, and get hit in the rear.
