a guess from your screenshot is you have 26 pop pts and very few loyalty generating buildings. Remember the welfare bonus (ie using pop in culture to generate loyalty) is capped at 30% (if I recall) of the base value. Your base value is very low so the +30% is not really helping.
I'd get your pop out of culture and into infrastructure and generate some more useful buildings.

well I can't work it out exactly due to how you are showing the info but lets assume all your 26 are your own nationality.
so from 6.4.3 of the manual and the 1.2 patch notes, you get
4*2 = 8
4*3 = 12
4*4 = 16
2*5 = 10
2*6 = 12
2*7 = 12
8*8 = 64
so that is 134 loyalty penalty. I assume you've got some slaves so the baseline is higher.
You have what looks to be 4 culture buildings (not clear so I am guessing), some of those may offset some of this lack of loyalty, but you've not got enough.
At its simplest 4 culture buildings in a region with 26 pop is a recipe for disaster.
Solve it by dumping an army on the region to avoid too high a revolt risk, emphasise infrastructure, get some of those (9?) building slots in use for the the loyalty buildings. Edit you won't improve your welfare bonus till you start to generate a higher baseline of loyalty.

you build up disloyalty by the number and type of population - your own ethnic group, other ethnic groups and slaves.
the example above is what happens as you have more & more of your own ethnic group. As you add more, they add more potential disloyalty - you need to offset this with suitable buildings and/or decisions
