Cohesion Double Drops

Question for those in the Know:

I seem to recall reading somewhere (or perhaps it was the video series), that double cohesion drops occur when a unit loses badly and then gets a modified cohesion test result of 2 or less. Do i have this right? Thanks.

It happened in my SP games once or twice that an undisrupted unit broke on impact- a triple drop!

Is this even possible, or was that a (very rare) bug?

Are you sure it wasn't autobreak or flanking cohesion drop followed by double drop on impact?

Not a bug. As the others say, it can happen from a flank charge, with automatic drop on contact, then double drop after losing the impact. It can also occur if the unit autobreaks.
