How are promotion points calculated?

Napalm update.
Veteran.
Game 1: DV. Promoted to CPT (straight past 1 Lt), although the end screen said 1 Lt.
Game 2: DV. Promoted to LTC (straight past MAJ), although the end screen said MAJ.

Can you get enough promotion points in one game to skip a rank?

Yes, I got promoted to Colonel and Brigadier General from Lieutenant Colonel in my last game. But from what I have seen the only difference between the ranks is the reset timer for airstrikes.

The only difference between the ranks is indeed the air strike counter.
