+3 init, but 50% of casualties fired back

I thought I understood the initiative system. With +3 relative initiative, I would have expected 75% of the casualties not to fire back. But this happened, so my understanding would appear flawed.

It is 20% advantage per point so +3 is 60% of 4 strength points, rounds to 50% or 2.
