this might be a stupid question, but I am questioning the usefulness of my "first aid hero". It seems the infantry units with "first aid hero" gain experience slower. Does the hero add "green" replacements? Or Elite replacements?

Some time ago I started a thread asking players which hero they thought was the worst, and my vote went to First Aid. I hate it. It was a good idea, but only 1 strength point per turn is worthless most of the time. For it to be valuable it should have given something like +3 to +5 str points per turn.
To answer your direct question, if you use it to replenish an infantry unit who has taken a beating, then yes, that is time it has been pulled out of combat - and opportunity to gain experience - to get free replacements. That's just not efficient, especially for a unit that suffers heavier and more frequent strength loss than other types. Plus, they aren't expensive like a good tank or other premium unit to be worth it.
Otherwise, no, they do not change the experience level of your unit so First Aid is like receiving Elite Replacements. I usually sell mine if I get one.

U forget about the transports. When an infantrie has an organic transport attached the prestice value is added to the value of the unit, making even early heavy/pioneer units equal in prestige to early tanks. The gap starts later in war when 1k prestige tanks start showing up.

I agree, that most of the time 1 strength per turn is not a good deal. But sometimes there is time, particularly towards the end of a scenario when not all units might be needed anymore. And it does save a little prestige as long as there are more hero slots than heroes avaiable

I do understand this, but the extra core slots that it cost often cause me to stick with truck transport for most infantry units. Halftrack transport are not needed in the Western theaters where there is an abundance of paved roads. while a t38 is 220 prestige.

This stays like that until u get Tiger I and Panther A. But even then it´s still on paar with Pioneer43 + SdKfz.

I see, and I still understand why it's not a big issue for me. I have never been one to go all Pionere infantry in my core for a couple of reasons. First, for personal flavor reasons going all the way back to Panzer General I like to build semi-historical cores based on divisions. Each division usually only had 1 Pioner battalion in it. Therefore, my core in AO usually consists of only 3 "divisions" and therefore only three pioniere. The second reason is because they are also more slot costly than the other types, I often do not deploy all of my three pioniere units anyway. Halftracks give the attached unit +4 soft attack when dismounted. I use the spanish Azul for my FirstAid, and I add the Cautious hero (added defense as harmed) for a passive-aggressive:
It attacks only at full strenght, then once damaged sticks as defensive where it will be healed one point, attacked or not.
And yes, as it almost doesn't attack, the exp gain is slow...
