Sources of cohesion drop?

The manual says:

"... Moreover, a unit cannot drop cohesion twice in the same turn from shooting, nor
from impact close combat, nor continuing close combat. (But can drop once for
each, and in some circumstances can double drop)...
"

1. Which circumstances can sometimes cause a double drop?*
2. Could this happen from shooting as well as close combat?

* Aside from: "Also, a unit that is Fragmented may break (without waiting to be contacted) if charged by another unit, even if it became Fragmented as a result of a previous impact close combat this turn."

Also, it says above:

"... a unit cannot drop cohesion twice in the same turn from shooting, nor
from impact close combat, nor continuing [melee?]
close combat...
"

3. Does this mean that cohesion in the same turn could drop first from shooting, then from impact, plus from melee?
4. I.e., potentially three drops?

If it has "lost badly" and
1) Scores low enough to double drop.
2) If it drops once, then tests for a further combat with addional modifiers that take it below the "double drop" threshold.

Note that "lost badly" is not for each individual combat, but takes into account all close combat in the turn so far.

Also note that the same random roll is used by the unit for all CTs for close combat in a turn. Likewise all CTs for the unit being shot at in a turn use the same random roll, but this is separate from the close combat roll.

No

No only two:
1) From shooting
2) From close combat (although this could be a double drop as outlined above)

That sentence could be better worded.
