How can we gain more command points? It seems that more only become available when units are destroyed, just sufficient to replace them.

That's at the discretion of the scenario designer. It's not something that the player has control over unless the designer has included a provision for gaining (or losing) command points depending on what happens during gameplay. Same thing for resources, by the way.
