Why can I sometimes not get special equipment like ajax's banner. In my first game, I got it. Then I restarted and it never came up as an option. Also, my friend was playing and he got some stuff I'd never heard of.

ehh, all that hero gear has a requirement or requirements, a skill or skills you need to get to a certain level.

From what I recall, the unit needs to reach approximately Level 25 or 26, and have certain promotions as prerequisites, before the option appears. Different types of special items are available to different types of units, so it all depends on which of your units reach that level with what promotions.

Considering that I try to distribute experience to lower-level units by positioning them to make the "easy" kills, while the hardened veterans "take the bull by the horns", I don't end up with a lot of units at that level until the last few missions, and then a large number of them all get the same options at roughly the same time. That doesn't give me enough time to rate the effectiveness of some of them, other than Ulysses' Bow, which effectively turns your archers into modern artillery batteries. I may have to try to get a unit or two promoted sooner next time around, just so I can test those "uber-weapons" against a few poor unsuspecting elephants.
