It seems that just as they were historically Civil Wars are a big deal in the game with tribal nations (at times of succession) , the Diadochi states and an expansionist early Rome particularly susceptible.

If a player loses such a Civil War is that game over or do they continue running the new government (whose decadence would be much reduced due to the govt age being reset ) ?

you have to survive with at least one region owned by your faction. So if Rome has a civil war and one of the Usurpers grabs the entire realm (or at least all your regions) you are out of the game ... but your legacy lives on (you may still win as a result).
