I was trying to get the Distinguished Service Medal achievement. Played USA Sandbox on Normal, landed a Gemini Direct Ascent on 1969.2, but achievement didn't trigger. Maybe it has to be in USA Campaign?

Spot on, you need to do it in the USA campaign. The achievement doesn't work on sandbox mode.
