Failures?

Nice to see the random events working well in the game now, and good to see the chance of them occurring has been reduced to a reasonable level. They work well now I think and add a bit of an unforseeable element to each game which plays well.
But I was wondering if there is any chance of getting the reasons for the mission failures into the game at all? They list seemed to be coming along when I last was involved, but they don't seem to have made it into the game as yet. Has any progress been made on them? Or did they end up being too big a job considering the variety of different ones required for all the missions? My feeling was they were just getting far too complex for what they needed to be.
Would it not be possible to keep them relatively simple and just do a list of failures for each type of mission stage? For example a list of perhaps five different failures for each of the countdown, launch, ascent etc parts, that could be relevant to all of the various missions types both Soviet and NASA? That would keep it relatively straightforward maybe. They would all be critical failures because minor issues seem to be fixable in the game now (which is a really nice touch).
