The Voskhod Duration IV mission remarks that it is levelling a penalty because I have not completed Duration IV yet.

I have completed duration I and II using Vostok, so I am skipping Duration III and should be punished for that.

Well spotted! It was a bug in the spreadsheet. You'll get the fix in tomorrow's patch. I've checked the rest of the entries and they seem to be OK.
