[SUGGESTION] - Random personnel accidents

One thing taht has bothered me with BASPM is that I rarely if ever employ "backup" staff, in clear contrast to how RL agencies worked.
So here's a suggestion again aimed at increasing realism, but at the same time adding an element of uncertainty:
1) Accidents in astronaut advanced training. Should not be too frequent, but still possible. They may leave the astronaut injured (not available for 1-3 seasons spent healing) or even or rare occasions dead (like the RL accident of Gagarin).
2) Other staff accidents. Should be very rare, but not entirely absent and are checked for randomly at the end of the turn. Accidents in everyday life or sudden ilness do happen, Korolyev's death during surgery is a good RL example. Again, these in game terms ould result in periodic absence due to injury (or depression, family tragedy etc) or death.
Overall these should be quite rare events, but should force the player to keep some backup personnel just in case.
