What happens to one of your cities when the enemy take it? Anything? Is it random destruction of building(s)?

I eventually took over a city in the south but could build nothing in it; all usable land was turned into those mounds of rough useless stuff.

I believe you are correct, that there is random destruction. I imagine that it clears over time - but basically is there to prevent immediate building on a conquered city.

I don't know that I have ever lost a city, so I can't be sure... ahem.

Yes you'll get a random number of buildings replaced with ruins - I think about 50%. Ruins remain until built on.
