How Are Heroes Awarded

Looking for a simple explanation of how heroes are awarded. Is it based upon destroying a unit with a certain level of experience? Is it more likely later in the war or is it tied to the level of difficulty selected? I am playing on Field Marshall and don't seem robe getting heroes like I did on the easier levels.
