Cav assault outside command - multiple CMT

I had the case of a spent cavalry unit, outside of command range, attempting an assault.
The wording on page 28 for assaults outside command range reads, "If the assault would normally require a CMT then 2 command points must be expended". This implies - to me - that one spends 2 command points but only rolls for 1 CMT....not 2 CMT but I could be wrong it might require 2 CMT - so, in the game, just to be safe I rolled the CMT twice.
I'm trying to find the wording that says how, in general, one handles the situation when multiple CMT conditions apply to the same move. Example, what if the following applies:
Assault through friends who are skirmishers, artillery or part of the same division
Assaulting when disordered or spent with units that are not guard, shock or impetuous
Any attempt to assault when out of command range
How many command points and CMT are required?
