I noticed this behaviour also in version 1.06, so it is most likely this behaviour is wanted. Unfortunately I was not able to find any post regarding to this behaviour so it has to be absolutly normal for all other users. Sometimes when a bomber attacks a clear city a new blank flag appears. In that case my bomber attacked Basra (14, 10) and a message was displayed that the citiy has been neutralized. A blank flag appears and for capturing this "new" flag normal reward is given (50 pp), so far so good.
I only wonder: why can players generate new flags in cities?

This has actually come up before, has to do with strategic bombers attacking city tiles with no flag.
Back then, it was such a minor issue (doesn't happen 100% of the time, doesn't cause any game breaking issues, not viable for prestige exploting) that it wasn't worth a patch or a hotfix. Might take another look now though before AK release, we'll see.

But they can't normally generate flags from hexes without flags, which is what the OP is saying.

Now that I think of it, I think the same thing happened to me in the same hex one game.

I play pretty fast so I can get through all the maps. I don't always notice everything when I move that fast.

Yes, this is a pretty old issue. But fixed now.

RC2: Works for me (while I can't attack empty cities without flag anymore).
