What counts as close/dense terrian?

I did a couple of searches, but couldn't find what I was looking for. Here is what I know for sure counts as close terrian.
Wood
Hills
Cities
Mountains
Hedgerows?
Marshes?
Anything else I missed?

What about frozen swamps or river hexes?

Frozen terrain does not change its "close" status, it only affects unit movement.

So, if a tank attacks infantry in forest, the tank uses soft attack value vs the infantries ground defense?
On the counter attack, the infantry uses the hard attack value vs the tanks close defense?

Yes. If you Ctrl+click your attack target you will get a popup window which will show attack and defense ratings used to resolve this particular combat, among other things.
