Question about bonus difficulty levels

Shall one finish the campaign by any path (e.g. with Germany scenario), or only by one ending with USA conquest, to unlock this bonus difficulties?

AFAIK, you need to win the USA West scenario decisively on Field Marshall difficulty level.

You can also cheat-win your way through the campaign on Field Marshal if you want to have these difficulty settings right away.

I fail to see the point though, if you don't manage Field Marshall you definitely won't manage the bonus levels

No, only different modifiers.
Guderian lowers the number of turns you have to complete a scenario or to achieve a decisive victory
Manstein sets enemy unit strength to 15
Rommel reduces your number of core units and prestige
I am not sure which one is the hardest I personally think Guderian makes decisive victories in some scenarios extremely hard.

Rommel seems the most challenging.
