Ambush's and Line of Sight...

I have always been confused as to what "line of sight to enemy skirmishers" means, or exactly how visibility works, outside of the effects specified by terrain.

I have seen some pretty funky things done with ambushes, and I would just really like to understand how this works in both FoG:A/M and FoG:R.

I think you will need to spell out your areas of confusion, otherwise it is hard to know how to answer this.

Well I have seen people put a unit of medium foot in an open field in ambush, and that doesn't seem to be right to me as they should be visible in most situations.

I guess what I don't understand is whether or not there is a range that troops can see up to, when it is NOT specified by a terrain feature?

Is visibility based upon the same line of sight as specified in shooting?

Yup. See the chart on P.169. Nobody can ambush in an open field.

There is no maximum distance beyond which troops cannot be seen in open terrain, nor do other troops block visibility. (In reality open ground is not usually perfectly flat)

Not exactly. Troops are visible if you can draw a line from any part of the "seeing" BG to any part of the "seen" BG without going through terrain that blocks visibility. The chart specifies this for different terrain types - some types only block line of sight if the "seen" BG is actually in the terrain, others block line of sight even if the "seen" unit (or "not seen" in this case) is beyond them.
