With byzantine if demesne size is 150(size for realm change) or more will that increase chances for civil war or give more negative effects?
If player goes over max demesne size (for authority gain) only negative effect is lower authority gain?

The two main ways to increase demesne limit:
1) Progress government to next level.
2) Assign nobles to govern domains who have high stewardship (There are some limitations for this for minor factions).

If you go over your allowed demesne size then you are correct in that it will impact authority gain. The snag is that it is your authority score relative to other nations that impacts if you are in the progress tier and hence can, over time, improve your government.

Others can comment on the issues regarding Byzantines as I've never played this faction.

Suffering authority is a very serious problem, that has secondary effects all over the place. You are limited in expansion, you get worse or less events of some types.
You can try to make a vassal, give regions away to people who likes you, build more auth buildings, always win battles etc.

It also means a minor but not insignificant penalty for the odds of someone trying a coup. Basically, it's not just "I lose more Authority, I'll compensate with more castles, etc."

Is coup when only ruler is killed and new character becomes ruler?
When civil war happens (too many regions), I still keep all governors but when ruler is killed I lose like half characters (generals or governors).

Required size for realm change is that requirement to progress when all tokens are collected?

Yes

I can only speak from experience on this, so there could be other aspects. I have seen 2 ways that a CW happens...a low loyalty general in command of a significant army, and a low loyalty governor in a low loyalty province. Most CW I have personally experienced are not complete breaking apart of my empire...though I am sure this is possible. For instance the BYZ probably experience some hellacious CW's.

Correct

Thank you for help.

I lost big army few times because general did have low loyalty so I only use generals with 100 loyalty to lead big armies in order to keep my armies when civil war happens but governors with low loyalty did not betray for now.

Civil wars will exhaust you when you play byzantine if you expand early because you already start with too many regions and new civil war can start before previous ended so many factions can appear and diplomacy is not working with them so its very important to stay below size limit.

You're welcome. Generals and governors are completely safe if they are 75 loyalty and above.

Playing the BYZ is a monstrous effort! They are built to lose territory before being in effective condition to expand.

But remember both Byz and Seljuq (and maybe others) lose some loyalty over time.
