Royal Marriages, heirs, and what happens if there isn't one?

Is the Royal Marriage Regional Decision the only way to get your heirs married?
I am in the fairly precarious position (for medieval times anyway), of my rulers only heirs being female, and the queen being beyond childbearing age (she's 49). So the game reports I have no valid heir, and 0% chance of a newborn.
What are the practical implications of this? Will the eldest daughter inherit? If so, why does it say I have no valid heir? Will one of my nobles take over? If so, which one? Is it game over? I doubt this, given that I've read about dynasty age resetting if your line dies out (which suggests its not necessarily terminal). But there is no information I can see on what actually happens.
Neither daughter is married to my knowledge, I understand the Royal Marriage decision would remove them from the game, but as I mentioned above, its unclear if this is the only method of marrying a character.
This issue has made me very aware of the scarcity of information provided to the player in FoG:K, and although I've been enjoying it a lot. If I had to give a criticism of this game, it would be the severe lack of genuinely informative and useful tooltips, and that the manual is frankly next to useless, with its severe lack of actual detail of mechanics. Another great example of this is spy networks. What is the point of even mentioning them in the manual if what is printed is all you are going to say?
